The IRS will be sending out only three 1040 forms: 1040, 1040-A and the 1040-EZ. This means public libraries will receive NO federal instruction booklets, publications or other forms for distribution to the public.
Again, please note: FEDERAL INSTRUCTION BOOKLETS ARE NOT BEING RELEASED TO LIBRARIES THIS YEAR! We will have an instruction booklet available for copying (10 cents per page) or for you to use in the library. You may call the IRS (1.800.829.3676) directly to request an instruction booklet be mailed to your residence.
The State of Ohio will ONLY be sending INSTRUCTION BOOKLETS for public distribution. Individual forms may be downloaded or copied.
